Receiving a Service from Us – Terms and Conditions

Forward Carers is a not-for-profit Social Enterprise whose aim is to improve the physical and mental wellbeing of unpaid Carers, young and old, including parent carers. We run a number of services for Carers, including this Carer Friendly Wiltshire website, to provide online support in your caring role. By using the website, completing the registration form via this website and/or accepting a service from Forward Carers, you are agreeing to how your information will be used, stored, and accessed in line with the below terms and conditions.

What Information Do We Hold About You?

We record a range of information about you, including your contact details, demographic details, health and wellbeing information you choose to share with us, information about your caring role, and some details about the person you care for. We try to minimise the amount of information we hold to ensure we only collect information that is relevant and required for you to receive the services you have requested.

Where Do We Get the Information From and What Do We Do With It?

When you register for your Carers ID card and/or complete light-touch Assessment registration forms, we collect personal and/or sensitive information about you. This allows us to register you for support and provide your card. We will also send you relevant information about the support you are entitled to, via email. If you wish to, you can unsubscribe from your Carer Friendly email at any time by clicking the link in your email.

How Your Information Is Stored Securely

Your information will be saved and stored securely on Forward Carers’ management systems. We are committed to ensuring that your Personal Information is secure. To prevent unauthorised access or disclosure, we have put in place suitable physical, electronic, and managerial procedures to safeguard and secure the Personal Information we collect. These steps include the following:

· Data minimisation

· Password best practices

· Security best practices concerning devices (PCs, laptops, mobile devices), online accounts, website hosting, physical access, and storage

· Staff training and accountability on data protection

Any information you provide to us is confidential and kept in accordance with the General Data Protection Regulations (GDPR).

We have policies in place to manage a personal data breach, should one occur. In the event of a data breach, we would promptly notify you of any unauthorised access to your personal information.

How Long Do We Keep Your Data For?

We follow the General Data Protection Regulations (GDPR) to determine how long we should keep your personal data, and we only keep it for as long as is ‘reasonable’ or when there is a legal requirement for us to do so.

Sharing Your Information to Provide Support

Data Controller

Forward Carers operates the Carer Friendly Wiltshire website on behalf of Wiltshire Council and NHS Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board (the ICB), who are the Data Controllers. Therefore, your data will be shared with them.

Data Processor

Forward Carers acts as the data processor for all data collected on the Carer Friendly Wiltshire Website

Data Sharing with Third Parties

Your information may also be shared with the Carer Support services in Wiltshire to ensure we can offer joined-up support.

Disclosing Your Information

We won’t share your information with any other organizations for marketing, market research, or commercial purposes, and we don’t pass on your details to other websites.

We do not sell personal information to third parties. But we may pass on your personal information if we have a legal obligation to do so or if you ask us to.

Your Rights

You can find out what information we hold about you and ask us not to use or delete that information we collect, at any time. You can contact us via our ‘Contact Form’ on the website to request this or email us at info@forwardcarers.org.uk. The only exception to this is if we must keep information if we have a legal duty to do so.

Business Data

If you are an organisation that joins our Carer Friendly Community and/or provides an offer to the Carer ID Card scheme, some of the information you provide may be used to create a directory listing on the Forward Carers website and to promote your organisation via social media.

The information that is stored about your company by the website, if approved by you, is limited to:

· Logo or image

· Address

· Website Address

· Phone number

· Your offer

· Name of business
